Please complete and submit this form to enroll a participant in an Insights Discovery. Insights Discovery Sessions: four weeks prior to the session enrolled participants will receive a link to complete an online evaluator. The responses to the evaluator will be used to create a profile. Two days before the session the personalized Insights Discovery profile will be sent electronically.
